
The Community Education Coordinator plays a critical role in advancing FAN’s statewide education and prevention efforts, which aim to reduce the stigma of addiction, prevent prescription drug misuse, and support individuals and families impacted by substance use.
This individual is responsible for planning, delivering, and tracking substance use education and training sessions, facilitating school-based and community-focused prevention programs, and coordinating the distribution of life-saving resources like naloxone and drug disposal systems. The ideal candidate is a confident public speaker, passionate educator, and relationship-builder who thrives in dynamic community environments.
Resposibilities will include:
● Deliver presentations and trainings across Michigan on topics such as prescription drug misuse, overdose prevention, naloxone use, and addiction stigma across schools, health systems, law enforcement, and community organizations.
● Facilitate FAN’s youth prevention programming, including sessions on early adolescence risk factors, healthy coping strategies, and early substance use warning signs.
● Engage students through age-appropriate discussion, relatable examples, and storytelling from people in recovery.
● Work with school personnel and administrators to customize session delivery to meet school schedules, academic calendars, and specific student needs.
● Establish a safe and supportive environment for dialogue with youth, while identifying opportunities to connect students to school support staff, crisis lines, or recovery resources as appropriate.
● Track and evaluate student engagement and program impact through pre/post surveys, educator feedback, and attendance records.
● Support the expansion and continuous improvement of FAN’s prevention programs by contributing feedback from the field and helping to pilot new strategies and materials.
● Use interactive tools such as testimonial videos, click-and-play lessons, and real-life recovery stories in youth sessions.
● Develop, adapt, and improve educational materials and presentations.
Community Engagement & Events:
● Attend and coordinate community tabling events and health/resource fairs to raise awareness of FAN’s services.
● Build and maintain relationships with schools, coalitions, healthcare professionals, law enforcement, and other stakeholders.
● Collaborate with law enforcement and community organizations to coordinate school-based programming delivery.
● Promote FAN initiatives through direct outreach and participation in local and regional events.
Resource Distribution
● Coordinate the community distribution of naloxone and Deterra Drug Deactivation Bags at trainings and events.
● Maintain accurate records of units distributed and individuals trained.
● Partner with the creative and communications teams to create outreach materials and expand visibility.
Coalition & Workgroup Support
● Support and sustain local and regional workgroups focused on education and prevention.
● Facilitate stakeholder meetings, take and distribute meeting notes, and support action planning.
Administrative & Organizational Duties
● Represent FAN professionally and consistently uphold the organization’s mission and values.
● Work closely with FAN staff, contractors, and volunteers to support cross-program collaboration.
● Perform other duties and special projects as assigned.
Requirements
● Bachelor’s degree in human services, public health, education, communications, or related field.
● Minimum 2 years of combined paid or volunteer experience in community education, youth programming, or substance use prevention.
● Comfortable with public speaking and facilitating in front of large or diverse audiences.
● Must be available for evening and weekend work as needed.
● Valid Michigan driver’s license, reliable transportation, and proof of auto insurance.
● Ability to pass criminal background and driving record checks.

The Peer Recovery Coach (PRC) is a key part of FAN’s Detroit Quick Response Team, providing immediate and ongoing support to individuals impacted by substance use. This role is rooted in lived experience, compassion, and the belief that recovery should be met with dignity and without judgment.
This position combines the principles of peer support and community outreach. The PRC works side-by-side with an Outreach Specialist, responding to overdose-related calls, community referrals, and follow-up needs across Detroit.
FAN’s QRT model is based on proven strategies used across the country to reduce overdose deaths and promote recovery by meeting people where they are—on the street, in their homes, or wherever help is needed most.
Responsibilities will include:
- Engage individuals in a trauma-informed, person-centered way during field outreach, including overdose follow-up visits.
- Offer emotional support, encouragement, and a connection to care that honors all paths to recovery.
- Link individuals to treatment, harm reduction services, peer support, and basic needs resources.
- Share your personal recovery experience to foster trust and demonstrate the reality of recovery.
- Help remove barriers to recovery (e.g., transportation, documentation, stigma).
- Document interactions and outcomes clearly in the case tracking system.
- Maintain up-to-date knowledge of FAN services, community resources, and trends in recovery and harm reduction.
Requirements:
- Minimum of 3 continuous years in recovery.
- High school diploma or GED.
- Valid driver’s license and insured, reliable transportation.
- Must pass a background check, sex offender registry check, and a drug screen within 48 hours of hire.
- Completion of CCAR Recovery Coach Academy (or willingness to complete the 5-day course before starting).
- Completion of 16-hour CCAR Ethics within 4 months of hire.
- Strong understanding of trauma-informed care and harm reduction.
- Ability to maintain boundaries and model professionalism.
- Experience with community-based outreach or street outreach preferred.

The Community Response Navigator for the Detroit Quick Response Team (QRT) plays a vital role in supporting daily operations and ensuring timely response to overdose incidents and public safety referrals. Acting as the central point of coordination, this position organizes outreach activities, manages real-time data, and supports the team both in the office and in the field when needed.
Most shifts will be scheduled from 6:00 AM to 2:00 PM, with rotating weekend coverage required. Flexibility in hours is essential, based on the needs of the team and the community.
Responsibilities will include:
Overdose & Outreach Coordination
- Monitor and enter overdose data into the internal tracking system.
- Contact area hospitals to confirm patient status for follow-up outreach.
- Prepare and prioritize QRT visit forms based on urgency (non-transports, recent overdoses, follow-ups).
- Re-route teams in real time based on incoming overdose alerts or hospital discharges.
- Track and update shift activity reports using internal spreadsheets.
Operational Support
- Participate in outreach efforts when additional support is needed.
- Assist with hospital responses and overdose follow-ups in the field.
- Support the day-to-day functioning of the Detroit QRT office, including organizing materials and keeping team tools updated and accessible.
- Create and assemble harm reduction/outreach kits.

The Case Manager for the Detroit Quick Response Team (QRT) provides intensive, long-term support to individuals impacted by substance use disorder, with a specific focus on co-occurring mental health challenges and barriers related to housing, income, or employment. This role is responsible for outreach, case management, transportation, and systems coordination aimed at improving participant stability and overall quality of life.
The Case Manager also plays a key role in managing follow-up efforts, tracking outcomes, and building strong relationships within Detroit’s criminal justice and housing systems to strengthen service connections.
Responsibilities will include:
Outreach & Immediate Engagement
- Participate in field-based outreach alongside DQRT team members following non-fatal overdoses or public safety referrals.
- Conduct home and community visits to offer support, assess needs, and build trust with participants.
- Provide harm reduction education and distribute resources such as naloxone, hygiene supplies, and referral materials.
Case Management & Coordination of Care
- Deliver person-centered, intensive case management for participants with complex needs, supporting long-term recovery and stability.
- Develop individualized support plans that address substance use, mental health, and barriers related to housing, income, or employment.
- Coordinate services across treatment providers, housing agencies, workforce development programs, and the criminal justice system.
- Provide transportation to treatment, housing, legal, and support service appointments to reduce barriers to engagement.
- Maintain regular contact with participants and offer consistent follow-up for up to 12 months.
Outcome Management & Quality of Life Focus
- Track participant engagement, service utilization, and progress using FAN’s case management systems.
- Monitor and document improvements in participants’ quality of life, including health, safety, housing, and purpose.
- Collaborate with leadership to analyze trends and contribute to continuous program improvement.
System Collaboration & Advocacy
- Build and sustain strong working relationships with courts, probation departments, shelters, behavioral health providers, and reentry programs.
- Serve as a liaison between participants and systems partners, advocating for timely, equitable access to needed services.
- Participate in local coalitions, multidisciplinary teams, and community initiatives aimed at reducing barriers and improving coordination.
Requirements:
- Bachelor’s degree in Social Work, Human Services, Criminal Justice, or related field required — or equivalent professional/lived experience.
- Minimum of 2 years of experience in case management, outreach, or behavioral health services.
- Working knowledge of substance use disorder, harm reduction principles, mental health supports, and Detroit’s housing and justice system resources.
- Ability to build rapport with diverse populations and respond effectively in high-stress or crisis situations.
